Failure to follow these instruction exactly will result in non-certification by the Secretary of State!
Required print format
- Print only on 8.5 x 11 inch white paper
- Set the printer margins to none or zero
- Print two sided, front and back.
- Make sure that the top is at the same end of the page
- Petitions not front-to-back will be rejected
County Designation
- Fill out the All Signers of this petition must be registered in WRITE IN NAME County
- Signers not living in the identified county will not be certified
Each Signer must fill in 5 items:
- Print Name
- Residence Address
- Signature as Registered
- City
- Zip
Declaration of the Circulator
- You, the Circulator, must fill in the Declaration of the Circulator completely
- Any omission or style errors will result in the rejection of the petition
Mail Completed Petitions
- Place in appropriate enclosure to assure undamaged delivery
